Completing the client intake form is an important part of beginning your work with Teresa Zuvela of Woodland Pathways Counseling. It helps Teresa to understand your specific situation and your desires for communication with any other health care providers you are working with. It also lets you know what you can expect from Teresa so your work together goes as smoothly as possible.

Woodland Pathways Counseling uses an online HIPPA compliant charting system for their client intake form. The online system is called simplepractice. New clients will receive an email from Woodland Pathways Counseling with a link to their chart to fill out the necessary paperwork for the first session.

If you are uncomfortable with the online system, paper forms May be sent to you via email. You will be required to have a payment card on file to pay for services.

The client intake form includes sections on the following topics:

  • Patient Registration
  • Psychosocial Assessment
  • Social Media Policy
  • Consent form including Confidentiality/Cancellation Policy
  • Disclosure and Authorization Form
